Craig Tiller 1b4e33029e Consolidate C++ proto files under src/proto 
- make the directory structure match the package structure
- sanitize the package structure (test --> testing)
- add auto-detection of proto file dependencies

Rakesh Iyer 4abe76eff5 Repro case for flow control bug. 
This is a repro case for a bug where if the client is sending
full throttle and the receiver is not able to keep up, the
client reaches a case where it doesn't make forward progress
anymore.
