The existing implementation was causing segmentation fault because
src/ruby/ext/grpc/rb_call.c:358 was trying to convert any value type other than
Array to String. The Array type is handled in first `if`. This change will cause
the Ruby code that sends non-string values to fail with a better message:
`ArgumentError: Header values must be of type string or array`

resurrect a deleted stream, causing all sorts of crash.

Fixes a multithreading bug in the ruby rpc_server
...
This fixes the current ruby server interop test flakiness.
- The interop tests recently changed so that clients access servers in
parallel, meaning that the interop servers handle multiple rpcs at once,
which is how this bug became visible
- since this change, tests run against the ruby interop server have
failed sporadically
- the problem was that the block in #loop_handle_server_calls that is
passed to the thread pool referenced a var in an enclosing block,
which resulted in requests being processed by the wrong handler
- this fix ensures that the block to be run on the thread pool thread
does not have any references to vars in the enclosing block

The encoding of the issuer field in this cert is now a PRINTABLESTRING

Make the server report monotonic times for deadlines
...
For very high performance systems, we're going to want to be able to
simply push the value reported from the server down onto clients.
If we report realtime now, then all wrapped languages are going to
assume it, meaning that such a change will be impossible later.

Corrects the cancel_after_first_response behaviour
...
- introduces a #wait method on the call operation view
- invokes #wait on a Notifier that is created for all operations
- ensures the Notifier is invoked if necessary whenever a client request
completes
- updates the interop_test to use op.wait before checking if the call
was cancelled.
