Added overloads for metadata add/remove operations when static index is known.

In several cases, we wish to add or remove metadata when we already know what
kind of metadata it is (e.g. we know we're dealing with the path, or the
authority, or the user agent, etc.).
In these cases, we do not need to re-compute the metadata batch callout index
since we know it a-priori. This saves us some branches and ALU ops spent
pointlessly re-computing these indices in several hot-path filters.
We do need the original methods where we do compute the indices in cases where
we're operating over a collection of metadata, but this is relatively uncommon.

Added specializations for grpc_mdelem_create.
...
In several cases, we create grpc mdelem structures using known-static
metadata inputs. Furthermore, in several cases we create a slice on
the heap (e.g. grpc_slice_from_copied_buffer) where we know we are
transferring refcount ownership. In several cases, then, we can:
1) Avoid unnecessary ref/unref operations that are no-ops (for static
slices) or superfluous (if we're transferring ownership).
2) Avoid unnecessarily comprehensive calls to grpc_slice_eq (since
they'd only be called with static or interned slice arguments,
which by construction would have equal refcounts if they were
in fact equal.
3) Avoid unnecessary checks to see if a slice is interned (when we
know that they are).
To avoid polluting the internal API, we introduce the notion of
strongly-typed grpc_slice objects. We draw a distinction between
Internal (interned and static-storage) slices and Extern (inline and
non-statically allocated). We introduce overloads to
grpc_mdelem_create() and grpc_mdelem_from_slices() for the fastpath
cases identified above based on these slice types.
From the programmer's point of view, though, nothing changes - they
need only use grpc_mdelem_create() and grpc_mdelem_from_slices() as
before, and the appropriate fastpath will be picked based on type
inference. If no special knowledge exists for the slice type (i.e. we
pass in generic grpc_slice objects), the slowpath method will still
always return correct behaviour.
This is good for:
- Roughly 1-3% reduction in CPU time for several unary/streaming
ping pong fullstack microbenchmarks.
- Reduction of about 15-20% in CPU time for some hpack parser
microbenchmarks.
- 10-12% reduction of CPU time for metadata microbenchmarks involving
interned slice comparisons.

Introduce C++ wrappers for gpr_mu and gpr_cv.

Introduce RAII wrappers in the grpc::internal and grpc_core
namespaces, and use them in place of std::mutex and
std::condition_variable.
Note that, since std::mutex is also used by the public
C++ headers we cannot introduce these wrappers in grpc_core.
Also, note that in grpcpp we cannot use gRPC core and vice versa.
So we had to duplicate the code, once using core_codegen_interface
and once using direct calls.

Introduce grpc_byte_buffer_reader_peek and use it for Protobuf parsing.
...
grpc_byte_buffer_reader_next() copies and references the slice. This
is not always necessary since the caller will not use the slice
after destroying the byte buffer.
A prominent example is the protobuf parser, which
calls grpc_byte_buffer_reader_next() and immediately unrefs the slice
after the call. This ref() and unref() calls can be very expensive
in the hot path.
This commit introduces grpc_byte_buffer_reader_peek() which
essentialy return a pointer to the slice in the buffer, i.e.,
no copies, and no refs.
QPS of 1MiB 1 Channel callback benchmark increases by 5%.
More importantly insructions per cycle is increased by 10%.
Also add tests and benchmarks for byte_buffer_reader_peek()

Remove extra semicolons after function definitions

We are planning to enable -Wextra-semi flag in our project but some
header files in gRPC have extra semicolons that violates the check and
blocks us from enabling the flag.
This change removes unnecessary semicolons in the code. Note that having
semicolon after the GRPC_ABSTRACT macro technically also violates the
check, but it's fine for us since they are not used in public headers,
and it will be confusing to have lines ending only with GRPC_ABSTRACT,
so I keep them as-is.

Reorder code to set num_frequently_polled_cqs correctly.
...
When there num_frequently_polled_cqs is non-zero (aka hybrid server),
we create non-polling CQs for the sync methods. But, since we
increase num_frequently_polled_cqs for callback methods
after creating the sync CQs, the sync CQs would not detect
a hyprid server, and will create a polling CQ.
This commit reorders the logic, so that we increment
num_frequently_polled_cqs upon detecting a callback service.
This lowers the context switches by double digit percentage
when using callback API.

In tensorflow, RPC client thread doesn't active release,
rely on process to cleanup. If process have already
cleanup the global variable(g_default_client_callbacks),
after that client issue a RPC call which contains the ClientContext,
then once ClientContext destructor called,
pure virtual functions call error is reported.

Specifically: if a request handling thread is in flight but scheduled
out when shutdown is called on the server, but it has already passed
the shutdown check, then when it resumes it will add a grpc_call to
the completion queue that is leaked. We fix this by explicitly freeing
such calls after all worker threads have shutdown.
To manifest the leak, run the end2end::ClientCancelsRequestStream
test repeatedly on the unpatched server implementation. About 0.5% of
the time, the leak will manifest.

Removing instances of exec_ctx being passed around in functions in

src/core. exec_ctx is now a thread_local pointer of type ExecCtx instead of
grpc_exec_ctx which is initialized whenever ExecCtx is instantiated. ExecCtx
also keeps track of the previous exec_ctx so that nesting of exec_ctx is
allowed. This means that there is only one exec_ctx being used at any
time. Also, grpc_exec_ctx_finish is called in the destructor of the
object, and the previous exec_ctx is restored to avoid breaking current
functionality. The code still explicitly calls grpc_exec_ctx_finish
because removing all such instances causes the code to break.

Add a GrpcLibraryInitializer to client_context.cc
...
It's feasible that a program be written/linked such that it only use
ClientContext from grpc++, which could end up with the other instances
of GrpcLibraryInitializer not ending up in the final binary. Add a
GrpcLibraryInitializer to client_context.cc to ensure that the library
is initialized. The primary side-effect of the library not being
initialized when only using a ClientContext is that the destructor for
ClientContext indirectly ends up trying to call through
g_core_codegen_interface when destructing its metadata, which is null.
